Two separate Text Boxes are required.
First text box to include the verbiage that is added to every notary acknowledgement section.
"This notarial act was performed with two-way audio/video technology using the Blend Signing Room."
In addition, a second text box is required to include verbiage that is for the state of ILLINOIS only.
"The Foregoing instrument was acknowledged before me using an Electronic notarization system provider on DATE by BORROWERS NAMES."
Note: If the Borrowers will be signing as trustee, use the following verbiage.
"The Foregoing instrument was acknowledged before me using an Electronic notarization provider on DATE by BORROWERS NAMES, TRUSTEE."
